Charity Sale Item Donation/Booth Registration Form

On December 10 and 11, CICS Foundation will be hosting a Community Charity Sale.

 

This event will help fund much needed services and programs to the community at large. Your support is very important to CICS Foundation and is essential for us to operate effectively. 

 

If you would like to contribute anything to the sale please read the description carefully and fill out the form to the best of your abilities.

Registration And Donation Rules
  1. Donors must fill out a form listing the names of the item, a brief description , proof of purchase price, a photo, and the suggest sale price. All prices will be determined by CISCF and will not be negotiated with donor.

  2. Donors who expects a tax receipt after donating items to CICSF, may suggest a price or provide proof of purchase price. CICSF will set the sale price and list the items by donor.

  3. All items should be clean before being donated to CISCF.

  4. If you are registering for a booth, a number will be sent to you shortly after registering.
